Counselor Verification

Counselor Verification Form for Current High School Seniors ONLY
Any applications received after March 15th will not be accepted.

This form must be completed by the applicant's high school counselor. All information should be as of the end of the seventh-semester. Schools using block schedules should provide a transcript as of December 31.

Counselor: Please attach an official seventh-semester transcript and copies of applicant's ACT scores to this form. Schools using block schedules should provide a transcript as of December 31.

Student: You are responsible for ensuring that your counselor has completed the form and attached an official seventh-semester transcript and copies of your ACT scores by the stated deadline. Failure to have the form submitted with your high school transcript and ACT scores render the scholarship application incomplete. Incomplete applications are not considered.
